This project will support ‘harder to reach’ vulnerable, socially isolated older people and carers in Leith, including those with disabilities, lesbian, gay, bisexual or transgender (LGBT) or who have been recently discharged from hospital. We will provide local ‘information stations’, encourage better links between older people with local services and provide practical help/volunteer support to better connect local older people to activities, groups and services available to them. This project has a primary focus on Prevention and Early Intervention which will enable older people to lead active lives in their local community and to remain in their own homes.
Delivery of an older peoples service (with volunteer support), to help reduce social isolation and loneliness experienced by older people in Leith, through increased levels of engagement. It will connect older people to community activities, opportunities and friendships regardless of their circumstances, ethnicity or disability, providing a ‘person centred’ service and enhancing their quality of life.
